Wellness Watch consists of a team of providers dedicated to the delivery of patient-centered skilled care, Home Health, and long-term health management, specializing in Geriatric and Palliative Medicine.

    We aim to provide personal care to patients who are in a vulnerable state and need direct and personal attention to meet their needs. As providers in the community, it is our responsibility to strive to bridge the gaps that exist in healthcare. The transitional period after an inpatient stay is when rehospitalization risk is the highest and when details can fall through the cracks.
